Hillsboro Pediatric Clinic LLC was the first pediatric clinic to be recognized in Oregon as a Patient Centered Primary Care Home ("Medical Home").

The Oregon Health Authority has set these Standards for Care to be a Patient Centered Primary Care Home:

• Access to care when needed
• Accountability for best possible care
• Comprehensive whole person care
• Continuity to help patients over time to improve health
• Coordination with others in the health care system to help patients get needed care safely
• Patient and family centered - our Care Team helping your child learn to be responsible for his/her overall health and wellness

What will this mean for your child and your family?

• Hillsboro Pediatrics offers extended hours Monday through Thursday evenings and Saturday mornings to provide an alternative to urgent care or the emergency room.

  • Telephone Advice Nurses are available during regular and extended hours.
  • Call coverage 24 hours, 7 days a week is available for urgent, non-life threatening questions.
• Care will be provided by your team. This decreases the number of different providers who see your child. In the unusual case your Care Team is not available, your child can be seen by another provider.
• If your children have special healthcare needs, a Care Coordinator (one of our RNs) will be assigned to work with your child's doctor. She will be the contact person for your questions or concerns between visits.
• If a child in your family is referred to more than one specialist, we will try to keep them within the same hospital system if possible. This can make it easier for your specialists to help if your child is hospitalized.
• Care will be patient- and family- centered. You as parents will help us develop goals of care to address your concerns, so the medical care makes sense for you.
