About Us:
We are a Medical Home. Hillsboro Pediatric Clinic LLC was the first pediatric clinic to be recognized in Oregon as a Patient Centered Primary Care Home ("Medical Home").
As a Medical Home, services are offered through Care Teams. Each patient is assigned a Primary Care Provider (PCP), and each PCP is a member of one of our Care Teams. You can expect:

Child Health and Development Interactive System (CHADIS)
Hillsboro Pediatric Clinic (HPC) has partnered with CHADIs to make it easier for you to complete questionnaires that are an important part of our standard of care. At selected office visits, you will be asked to complete questionnaires that help us track your child's growth and development. We will give you the questionnaires based on your child's age and the type of visit that they are scheduled for.
First Time Users: When HPC assigns you or your child a questionnaire, you will receive an email and/or text message inviting you to register at CHADIS.com. Click on the link in the invitation to complete the registration. After completing the registration you will have a username and a password that will allow you to log on to the CHADIS.com website. When you log into CHADIS.com, you can see and complete questionnaires placed there by your primary care provider.
Families with more than 1 Child: If you have already registered with CHADIS.com and you receive a new invitation email and/or text for another child, please log into your existing CHADIS.com account and enter the new invitation code to add that child to your account.
HPC will notify you to re-enter CHADIS.com by email or text when there is a new questionnaire to complete.
