Unfortunately, due to the state licensing requirements, if our staff is aware you are outside the state of Oregon, we will not be able to provide triage or advice. We appreciate your understanding of the requirements we have to follow, and hope all of your travels are safe, happy, and healthy!
All FMLA forms will be ready for families within 15 business days upon receipt. All other forms (PE forms, medical statements, etc.) will be ready for families within 10 business days.
