We are working hard to get her added to all of our insurance plans so that she can start seeing all of her patients and start taking new ones. If your child has not had a physical or Well Child Check in the last 12 months they will need to complete one prior to being cleared to play sports. Now is the perfect time to schedule your child’s Well Check to avoid delays in their participation. Our schedules are open for the summer- extended time during the evening is also available, schedule for a time that is convenient for you!

Make sure your child has had their Well Child Exam done in the last 12 months! If you need clearance for sports bring in your form as soon as possible. We have a standard Physical form here.
