ADHD Evaluation Process
If you would like us to evaluate your child for ADHD, please call us to initiate the process. However, our providers would like their patients to be up-to-date on their annual Well Child Check before setting up an initial ADHD Evaluation. If you’re unsure whether your child has had an annual Well Check, please call our clinic.
In order to provide an accurate diagnosis of ADHD, HPC has adopted a standard number of Vanderbilt forms to be completed by either the patient’s parents (or caregivers) and teachers. These forms must be completed and returned before we can schedule an appointment.
- 2 parents/caregivers
- 3 teachers
- 1 Youth Self-Report (for ages 11 – 18)
Once the required questionnaires are completed and returned, we will call you to schedule an appointment with your child’s Primary Care Provider (or the provider who will manage their ADHD visits) and a Behavioral Health Provider.
