“What does Medical Home mean for your child and your family?”

HPC offers services through care teams. Each Provider is part of either our Penguin or Dolphin team. Our Best Beginnings team is dedicated to care for all of our newborns for the first 30 days of life before transitioning to either the Penguin or Dolphin team.

Scheduling an Appointment

When you call in to schedule an appointment for your child our staff will do their best to get your child in with their primary provider however if he or she is not available they will be able to schedule you with another provider on the same care team. Our goal is to be consistent and decrease the number of different providers who see your child.

Special Health Care Needs

If your child has special healthcare needs, a Care Coordinator will be assigned to work with your health care provider. She will be the contact person for your question or concerns between visits.

Referrals

If a child in your family is referred to more than one specialist, our referral team will try to keep them within the same hospital system if possible; this can make easier for your specialist to help if your child is hospitalized.

Access to Care

HPC offers extended hours Monday through Thursday evenings and Saturday mornings at our Main Street location in attempts to decrease your need to utilize urgent care or the emergency room. We have telephone Advice Nurses available during our regular office hours and there is emergency call coverage 24 hours, 7 days a week for urgent, non-life threatening questions. Patient-and Family Centered Care

Care at HPC is patient- and family-centered. As a parent you will help our providers develop goals of care to address your concerns. Your concerns, questions, child’s needs and family priorities will be shared with the provider so that you and your family can be involved in making decisions about your child’s health care.

Care Team Goals

Our goal as your child’s care team is to get to know your child and your family situation. We will listen to your concerns and answer your questions. We expect that your child receives access to care when needed and always attains the best possible care.
